Immunizations 

We comply with Ohio Revised Code 3313-671 which requires a completed immunization record or exemption to be on file for every student within the first 14 days of admission into our district. Students entering 7th and 12th grade are required to show proof of additional vaccinations that are required at that time. Please see links below;

Screenings

During the school year, the district provides vision and/ or hearing screening to children in grades Pre-K, K, 1, 3, 5, 7 (vision only), 9 and 11. You will only receive notification if your child does not pass the screening according to guidelines set by the Ohio Department of Health.

Emergency Action Plans

For students with acute and/or chronic health conditions for special health needs requiring treatment and/or medication to be administered at school.

Valid for one academic year.
